Patients

Overview

To take the Asthma Adherence Pathway™ survey, you have to be enrolled in the program by your participating healthcare provider.

Your Access Code
To enroll or login for the first time, you will need an Access Code. Your participating provider or payer will give you the Access Code. You will use this code only once, when you first register on the website. After your initial registration, you will log on to the site with a Username and Password determined by you at the time of registration.

New User Registration

  • If you do not have an Access Code, contact your provider or payer to obtain one.
  • Click on either the "Take Survey" button or the "Log In" button on the Asthma Adherence Pathway™ home page.
  • Type your Access Code in the blank field under "First-Time User". The codes are not case-sensitive.
  • Click the button that reads "enter using the secure server".
  • On the Registration Information page, fill in all fields as directed
    • First Name
    • Last Name
    • Last 4 digits of SSN
    • Email address - if you do not have an email address, type "none@email.com" in this field
    • Username - a minimum of four characters, and a maximum of twenty
    • Password - a minimum of four characters, and a maximum of twelve
    • Verify Password - retype the same password. (Typing the password twice helps to ensure you don't accidentally type the wrong word.)
  • Make a note of your Username and Password and keep it in a safe place for later reference
  • Click the "Submit" button
  • Click the "Continue" link to move to the beginning of the Survey
  • When you are ready to begin taking the Survey, click the "Next" button on the "Instructions" page.

Returning User Login

  • Click on either the "Take Survey" button or the "Log In" button on the Asthma Adherence Pathway™ home page.
  • Under the "Returning User" section, enter your Username and Password in the appropriate spaces.
  • Click the button that reads "sign in using the secure server"
  • When you are ready to begin taking the Survey, click the "Next" button on the "Instructions" page.

Taking the Survey

Please answer every applicable question on a screen. When you are satisfied with all of your answers for that screen, please press the "Next" button near the bottom of the window to move on.

If you log out of the system before finishing or otherwise interrupt your Survey, your answers will be retained for 24 hours. If you return to the Survey within the twenty-four hour period, you will resume in the section you left off. If you return beyond the twenty-four hour period, you will be required to start a new Survey. (Note: You will not need a new Access Code to take subsequent surveys. You can continue to log in with the same Username and Password.)

When you complete the Survey, you will see a printable Summary screen. Please review the Summary, along with the complete survey results (you will see a link for this at the bottom of the Summary), and then follow your healthcare provider's directions regarding your next steps. If you cannot review your results thoroughly at the time of completion, be sure to print them for later reference, as the online result will only be available to you for a 24 hour period.

Payers & Providers General

Access Codes

All payers, providers, and patients and need an Access Code for initial registration. Access Codes can be used by multiple people, or they can be limited to a single registrant. The number of uses allowed per code is determined at the time the code is generated, and is entirely up to the discretion of the payer or independent provider who created it.

Each type of code (payer, provider, or patient) gives the user who registers with it access to specific parts of the Asthma Adherence Pathway™ website.

For example, someone who registers under a Provider Access Code can see survey results for all patients associated with him/her. Someone who registers under a Patient Access Code can take the survey and see the results of the survey within a 24 hour period. A patient cannot, however, see the results of other patient's surveys, or see the results of surveys they, themselves, may have taken previously.

Generating Access Codes

All payers, providers, and patients and need an Access Code for initial registration. Because the Asthma Adherence Pathway™ Access Code system was built to accommodate the many possible relationships between payers, providers, and patients, it is very flexible. This flexibility, without any established procedures, may lead to confusion about who is responsible for generating, documenting, and distributing Access Codes to the parties that need them. Accordingly, it is strongly recommended that you (as a payer or an individually enrolled healthcare provider) determine your policy for generating Access Codes as soon as your license is obtained.

There are various entities that can generate codes:

Payer codes can only be generated by the Asthma Adherence Pathway™ Site Administrator.

Provider codes can be generated either by a payer organization, or by the Asthma Adherence Pathway™ Site Administrator. No matter who generates a Provider Access code, that code and any provider who registers with it, is always tied to the affiliated payer, if applicable.

Patient codes may be generated by a payer organization, by the Asthma Adherence Pathway™ Site Administrator, or directly by providers (independent or payer-affiliated.) No matter who generates a Patient Access code, that code (and any patient who registers with it) is always tied to both the provider and payer.

Establishing a policy is particularly importantly for large payers with multiple providers and patients they would like to enroll in the program. Some payers may want to generate, document and distribute Access Codes directly to patients, without the providers' involvement. Others may prefer to have their providers assume this task for their own caseload. Still others may decide on some combination of the two. Under any method, the system is designed to maintain the connections between payers, providers, and patients.

Providers

Overview

You may enroll in the Asthma Adherence Pathway™ (AAP) in one of two ways:

  • Through a participating organization (i.e. "payer") with which you are affiliated. This could be a hospital, HMO, or other healthcare agency.
  • As an individual healthcare provider.

If you are not enrolled but would like to be, please contact your payer agency or Asthma Management Systems for more information.

Once you are enrolled, the Asthma Adherence Pathway™ website will allow you to:

  • Check the results of a specific patient survey
  • Edit patients' names and information and generate patient access codes
  • Review a summary of survey results for multiple patients
  • Read new information on asthma and its treatment

Your Access Code

Depending upon how you enroll (either as an individual provider, or through a larger administrative payer entity) you will be given an Access Code by either the Asthma Adherence Pathway™ site administrator or by your payer. New User Registration

Review Individual Patient Results

Log in to the site with your Username and Password. From the "Healthcare Provider Menu", click on "Patient Results".

The Patient Results screen will show you a list of all your patients who have registered on the site, even if they have not yet completed a survey. (The Asthma Adherence Pathway™ program associates your patient records with you via their initial Access Codes, so be sure to read the section on providing Access Codes.)

The list is restricted to ten names per page, so you may have to use the "Next" button near the bottom of the window to locate a specific patient. Once you've located the appropriate patient, click on the "Survey Date" dropdown field, then select the date of the survey you wish to review.

A printable Summary of the survey will appear onscreen. In addition to reviewing the Summary, it is recommended that you explore the complete version of the results to get the fullest picture of your patient's adherence profile.

Generate Access Codes For Patients

All patients need an Access Code for initial registration. Patient Access Codes can be generated directly by you. If however, you are enrolled under the auspices of a payer organization, they may also be generated by your affiliated payer and tied to your name. If you are enrolled under a participating payer, please check with them to find out who should generate codes, and the logistics of how those codes should be distributed.

If you need to generate Access Codes directly, log in to the site with your Username and Password. From the "Healthcare Provider Menu", click on "Patient Admin".

Select the "Access Codes" link.

You can give a patient an existing Access Code, or you can generate a new one. Either is acceptable.

  • Generate New Code:
    • Near the top left of the page you will find the "Add Access Code" link. Click on this link.
    • Enter the number of patients you'd like to assign to this code, then click the "Submit" button.
    • Your new code will appear onscreen; write it down and communicate it to your patient(s).

  • Use Existing Code:
    • If you have already created one or more patient Access Codes, you will see a list of those codes in the middle of your screen.
    • Choose a code that is still active (it's active if it has a number other than "0" in the Assigned/Remaining column.)
    • Write down the code and communicate it to your patient(s).
    • Be sure to keep track of how many patients you've given a particular code. The number in the Assigned/Remaining column only shows how many have not used their access code yet, but may not accurately reflect persons who have received that code but not yet used it. If you give an Access Code to more people than it's good for, the later users will receive an error message that they need to obtain a new access code.

Payers

Overview

Once your organization obtains a license to the Asthma Adherence Pathway™ , the website will allow authorized users in your organization to:

  • Add or edit providers' names and information and generate provider access codes
  • Add or edit patients' names and information and generate patient access codes
  • Check the results of a specific patient survey
  • Review a summary of survey results for multiple patients

Your Organization's Access Code(s)

To start, please designate one or more people as system user(s) for your organization. To enroll or login for the first time, your system user(s) will need an Access Code. This code (or codes) should be given to you by the Asthma Adherence Pathway™ site administrator upon execution of the licensing agreement.

Each authorized user will use the code only once, when he/she first registers on the site. After the initial registration, your system user(s) will log on to the site with a Username and Password determined by the user at the time of registration.

New User Registration

  • If the organization does not have at least one Access Code, contact the Asthma Adherence Pathway™ site administrator to obtain one.
  • Click on the "Log In" button on the Asthma Adherence Pathway™ home page.
  • Type the Access Code in the blank field under "First-Time User". The codes are not case-sensitive.
  • Click the button that reads "enter using the secure server".
  • On the Registration Information page, the system user should fill in all fields as directed
    • Company Name - choose from the dropdown list of existing payer companies, or type in a new company name.
    • Title
    • First Name
    • Last Name
    • Last 4 digits of SSN
    • Email address - if you do not have an email address, type "none@email.com" in this field
    • Username - a minimum of four characters, and a maximum of twenty
    • Password - a minimum of four characters, and a maximum of twelve
    • Verify Password - retype the same password. (Typing the password twice helps to ensure you don't accidentally type the wrong word.)
  • Make a note of your Username and Password and keep it in a safe place for later reference.
  • Click the "Submit" button.
  • You will be taken to a completion screen and then the "Payer Menu", from which you can select several activities, including reviewing individual patient survey results, reviewing group results, and generating Access Codes for both patients and affiliated providers. You may also register Providers directly (as opposed to giving them Provider Access Codes and having them register themselves).

Returning User Login

  • Click on the "Log In" button on the Asthma Adherence Pathway™ home page.
  • Under the "Returning User" section, enter your Username and Password in the appropriate spaces.
  • Click the button that reads "sign in using the secure server"
  • You will be taken to a completion screen and then the "Payer Menu", from which you can select several activities, including reviewing individual patient survey results, reviewing group results, and generating Access Codes for both patients and affiliated providers. You may also register Providers directly (as opposed to giving them Provider Access Codes and having them register themselves.)

Review Individual Patient Results

Authorized users must login with the Username and Password they selected when they registered. Once they get to the "Payer Menu", they should click on "Patient Results".

The Patient Results screen will display a list of all your organization's patients who have registered on the site, even if they have not yet completed a survey. (If you would like to narrow your search to patients associated with a particular provider, select the provider's name from the dropdown list in the upper left.)

The list is restricted to ten names per page, so you may have to use the "Next" button near the bottom of the window to locate a specific patient. Once you've located the patient, click on the "Survey Date" dropdown field, then select the date of the survey you wish to review.

A printable Summary of the survey will appear onscreen. In addition to reviewing the Summary, it is recommended that your authorized users explore the complete version of the results to get the fullest picture of a patient's adherence profile.

Generate Access Codes For Patients and Providers

An authorized user should log in to the site with his/her Username and Password.

From the "Payer Menu", click on either the "Patient Admin" or "Provider Admin" link.

Select the "Access Codes" link. You can give out an existing Access Code, or you can generate a new one. Either is acceptable.

  • Generate New Code:
    • Near the top left of the page you will find the "Add Access Code" link. Click on this link.
    • (For patient codes only) Select the provider associated with this patient from the dropdown menu.
    • Enter the number of users you'd like to assign to this code, then click the "Submit" button.
    • Your new code will appear onscreen; write it down and communicate it to the appropriate party.
  • Use Existing Code:
    • If you have already created one or more Access Codes, you will see a list of those codes in the middle of your screen.
    • (For patient codes only) Select the patient's provider from the dropdown list in the upper left. This will filter the list to show only the patient codes tied to that provider.
    • Choose any active code (i.e. any code with a number other than "0" in the Assigned/Remaining column). Write down the code and communicate it to the appropriate party.
    • Be sure to keep track of how many patients/providers you've given a particular code. The number in the Assigned/Remaining column only shows how many have not used their access code yet, but may not accurately reflect persons who have received that code but not yet used it.
